Insured Clients - MCHD is contracted with most major health insurance companies, including Medicare and IL Medicaid. Please bring a valid copy of your health insurance card at every visit.
Please note - Insured clients seeking confidential services may request to use the sliding fee scale in lieu of submitting a claim to their group health plan.
Uninsured / Self-Pay Clients - services available at no cost for eligible clients; see sliding fee scale below for more information.
