*Valid insurance card must be presented at time of visit and does not guarantee coverage of benefits or eligibility, nor does it guarantee payment for services rendered by the Morgan County Health Department.  For policy information, contact your insurance provider.

*Convenience fees for credit cards are 2.65% of the payment amount with a $3.00 minimum.
