BBO-c program goals
Reduce the rate of maternal and infant morbidity and mortality
Help pregnant and postpartum individuals meet their family's health and wellness goals
Connect pregnant and postpartum individuals to needed supports available in their community
PROGRAM SERVICES
Nurse Navigators provide support and assistance to eligible clients by helping:
Pregnant individuals establish care with an OB/GYN or other medical provider
Provide referrals to social service agencies and community resources, as needed
Conduct childhood developmental screenings
Perform routine physical assessments
Provide enrollment assistance for IL Medicaid
Offering health education during and after pregnancy
CLIENT ELIGIBILITY
The Better Birth Outcomes - Comprehensive Program serves clients who meet all of the following criteria:
Pregnant or postpartum individual up to 6 months after birth and/or infant up to 6 months after birth; AND
Household income less than 200% of the Federal Poverty Level (FPL); AND
The presence of at least one risk factor as noted on the Vulnerability Index
